Waiver and Release:
Please review the Waiver / Release below. Submitting your registration indicates agreement to Salem County Dance and Cheer Waiver and Release. I, the undersigned registrant/parent/guardian, do hereby grant permission for my self/child named in this registration form to participate in the activities of the Salem County Dance and Cheer studio. I hereby acknowledge that by attending and participating in yoga, dance, gymnastics, and/or cheerleading classes and events that there is a possibility of physical illness or sever/fatal injury to my child and I do hereby for myself and all others who might have a similar claim waive, release and forever discharge any and all rights and claims for injury, which may arise now or in the future against Salem County Dance and Cheer, its owners, operators, officers, agents, or representatives, the host School/Institution, and their representatives, for any and all damages which my child may sustain while attending and participating in the events.

Student Rules & Regulations:
Please review the Student Rules & Regulations below. Submitting your registration indicates agreement to Salem County Dance and Cheer Student Rules & Regulations. Salem County Dance and Cheer Student Rules & Regulations Be Prompt: All students should arrive to class 5 minutes before the schedule start time to ensure they are ready for class. Be Courteous and Quiet: Show respect for the other class in session and to the teacher who will need the class’s full attention. Proper Dress is Required: All students must wear a dance leotard and tights of any color. (Black leotard will be required for the studio photo in May.) Dance shorts may be worn over the leotard if necessary. Dance skirts may be worn in all classes except gymnastics. Please no baggy pants; they do not permit the teacher to see the dancers legs. Cheerleaders must wear stretch shorts and t-shirt. NO JEANS of any type are allowed for any class. Dance Shoes: All students will be required to purchase dance shoes from the studio. Students must wear dance, gymnastics, or cheer shoes during class instruction. Please bring your dance shoes to class in a dance/cheer bag to ensure they stay clean. Put students initials inside shoe with permanent marker. Do not wear street shoes on the dance floor. Bare feet or socks are never acceptable. Hair: All long hair must be up in a bun or ponytail for all classes. Short hair must be pinned at the sides to bring the hair out of the face. Attendance: All students should attend all scheduled classes. If you will be absent, you must call the studio prior to 4:00 p.m. and leave a message. Attendance will be taken in each class and awards are given at the end of the year for perfect attendance. You may make up a class if you are absent due to severe sickness or family vacation within one month of the event. All other absences will not be excused for perfect attendance awards. Four (4) consecutive absences will result in removal from class. Arrive and Depart from Class: All students must be picked up inside the studio. For the safety of your children, no student will be permitted to wait outside the building for his/her ride, or allowed to exit the building without a chaperone. If you permit your child to walk home, a written note must be given to the director. Drinks/Food: There is no need to bring a water bottle if you are not in class for more than one hour – there will be no break. If you are at the studio for more than an hour or an extended length of time, you may bring a water bottle and/or snack. NO SODA, JUICE drinks, or GUM is permitted in the studio at any time. Friends and Guests: Please do not bring friends or guests to class; they are a distraction to the class. They may attend during the friends and family weeks when all are invited to watch the class. Waiting Area: Students may wait in waiting area until his/her class is announced. Parents are welcome to stay in the waiting area during class instruction. Please be courteous and remain quiet. Do not keep disruptive children in the waiting area, as they will be a distraction. Respect: Students must show respect for each other and all instructors and parent volunteers during class time and during events. Discipline: A five-minute “time out” will be enforced for any student who disrupts the class. Problems: If there is a problem among the students or with an instructor, the director must be notified immediately. All Students Will Be Treated Equal.
